As a Care Coordinator you can see the positive impact of your work on the lives of our consumers by performing telephonic screening, triaging care reviews, referrals and service authorizations. You will act as the telephonic liaison with community-based organizations to assure timely and appropriate access to care for routine, urgent, and emergency referrals. Care Coordinators ensure continuity of care through contact with providers, CMO, Mobile Response, Youth Case Management and families.
A Masters Degree in mental health field (MSW, etc.) is required. This position also requires licensure in a mental health related field (RN, Ph.D., LCSW, LPC, LMFT, etc.) with 2 or more years of experience in a behavioral healthcare field providing services for children/adolescents required. Experience with child welfare, juvenile justice or providing community-based services to at-risk children and their families a plus.
Several work shifts are available (8:30 a.m. to 5:00 pm, 10:00 a.m. to 7:00 p.m. and 1:30 pm to 10:00 pm) along with free parking and so much more!! To apply for a Care Coordinator position in our Hamilton, New Jersey office, please follow the link below!!
